When we get this book we learn his condition and how depressed it leaves him feeling. He feels for his and Izzy safety and they increase security at the vineyard house. When Avery comes to visit she tells him she not doing good and don't want to have any more surgery. He promised to be there for Izzy when and if any thing happens. But he pushes her away since he don't want her to know the real danger she might be in do to his brother. So he hurts her and she leaves they both start not caring what happens they quit eating and sleeping and all. When he finally comes back to the United States he decides he got to try and get her back.
